Before calling, make sure that you have:

• your monthly statement form;
• your social insurance number;
• your telephone access code for the month.

Step 1

Dial the following telephone number:

Step 2

Enter your social insurance number.

Step 3

Enter your telephone access code (five digits).

Step 4

Wait for confirmation from the system that your monthly statement has been recorded.

How to find out your telephone access code

• Locate the seven- or eight-digit number on the front of your monthly statement (in the bottom right-hand corner, under your telephone number). This number is also shown on your claim slips.

• The last five digits of this number are your telephone access code. The code changes each month.
