These forms must be filled out by hand. Application for Service — General Information (static, , 370 KB), Appendix 1 — Information about Education and Employment Services (static, , 369 KB) and Appendix 2 — Application for Last-Resort Financial Assistance (static, , 517 KB).

You must then mail the forms to the nearest local employment centre (CLE).

You can also go to the nearest CLE to file an application and obtain any necessary information.

The CLE staff will make sure that you have all of the documents required for your application and refer you to services that could meet your needs. The Ministère de l’Emploi et de la Solidarité sociale has some 150 local employment centres (CLEs) and service points across Québec.

Before submitting your application for last-resort financial assistance, you can obtain a preliminary assessment using Simul Aide. Note, however, that this simulation may not be used for legal interpretation purposes and does not replace the provisions of the laws and regulations in effect.

Do all the forms have to be filed?

The Application for Service—General Information form enables the personnel at your CLE to determine your needs.

If the services in question are employment related, you must complete that form and Appendix 1— Information about Education and Employment Services.

If you want to apply for last-resort financial assistance, you must complete the Application for Service—General Information, Appendix 1—Information about Education and Employment Services and Appendix 2—Application for Last-Resort Financial Assistance forms.

Appendix 2—Application for Last-Resort Financial Assistance is used to determine whether you are eligible for last-resort assistance and establish the amount of your benefit.

Processing

Step 1

To submit an application for last-resort financial assistance, you must complete the following forms:

• Application for Service – General Information (3003A) (static , 373 KB),
• Appendix 1 – Information about Education and Employment Services (3003-01A) (static , 369 KB),
• and Appendix 2 – Application for Last-Resort Financial Assistance (3003-02A )(static , 517 KB).

Which you can obtain from your local employment centre. If you are unable to pick them up in person, the forms can be mailed to you.

Don't hesitate to ask the local employment centre (CLE) staff for help if you have trouble completing the forms.

Generally, your eligibility for financial assistance is assessed as of the day the CLE receives your application form. It is important to produce the form as soon as possible even if you do not have all of the required documents. The form can be completed on-site and filed immediately to avoid delays.

The documents required for the evaluation of your application are:

• Two pieces of ID, including each family member’s birth certificate
• Social insurance card of each adult family member
• Documents constituting proof of your income (pay stubs, proof of cessation of employment, employment insurance benefit pay stubs)
• Lease, municipal tax statement or other relevant documents
• Your updated bank books.

You may be asked to produce other documents depending on your personal, family or financial situation.

Step 2

If required, an officer will invite you to an interview, during which he or she will:

• Analyze your situation and discuss it with you.
• Determine whether you are eligible for last-resort financial assistance and whether you are able to undertake a job entry or re-entry process.
• Inform you of the Ministère de l'Emploi et de la Solidarité sociale's obligations and of your rights and obligations concerning your eligibility for last-resort financial assistance.
• Inform you of the services available and, where need be, direct you to various community resources (employment-assistance agencies, food bank, etc.).

Step 3

Once the officer has received all of the documents required and rendered a decision, you will receive a notice of decision in the mail informing you whether you are eligible for financial assistance.

If you are eligible for last-resort financial assistance, the notice of decision will indicate the amount of assistance granted for the month following your application and the name of the officer assigned to your file.

If you are not eligible, the notice will indicate why.
