Monthly assistance

The monthly statement is a form that you have to complete and return to your local employment centre (CLE) by the 15th day of each month. You have to provide a monthly statement regardless of whether there have been any changes in your circumstances (family or financial situation, etc.).

Monthly statement

The monthly statement is a form on which you indicate any change in your family, financial or other situation. As a rule, you must complete this form and return it to your local employment centre (CLE)New window. as soon as a change occurs or no later than the 15th of each month. This way, the Department can adjust your next monthly benefit to reflect your new situation.

However, if you receive social solidarity allowances, you need return this form only if there is a change to declare.

Monthly statement by phone

You may make your monthly statement by phone if you meet the following criteria:

  • You are an independent adult or the head of a single-parent family;
  • Your capacity for employment is unlimited or temporarily limited;
  • You do not have any changes to declare since your previous monthly statement.

Periods during which the service is available

Generally, you can make your monthly statement by telephone at any time of the day or night, from the 15th to the 20th of the month. However, on the 20th of the month, the service is available only until 4:00 pm.

Different dates in June and December

In June and December, you can make your monthly statement by telephone at any time of the day or night from the 10th to the 15th of the month.

Note: In certain circumstances, these dates may apply to other months as well.

Claim slip

The claim slip is a document that enables you to obtain:

  • certain prescription medication;
  • certain dental and optometric services.

Although generally reserved for adults or families who are last-resort financial assistance recipients, the claim slip may, under certain conditions, also be issued to a dependent child who does not live at home, an adult, or a family who does not receive last-resort financial assistance benefits from the Department.

Direct deposit

If you currently receive your last-resort financial assistance benefits in the form of a cheque, you can request to have them deposited directly into your bank account. This occurs on the 1st of the month or the working day preceding it.

Every month, you will also receive the monthly statement, claim slip and notice of deposit by mail. You must complete the monthly statement and return it to your local employment centre (CLE), as usual. If you fail to return it by the prescribed deadline or if the officer assigned to your file wants to meet with you to get information or check information contained in your file, you may be asked to go to pick up your cheque in person at the CLE.

Furthermore, all of the financial assistance to which you are entitled during the month is deposited directly into your account as it comes in. You will receive a notice of deposit for each amount.

Remember to sign the form and return it to your CLE. If you have a joint bank account, both authorized individuals must sign the form.
