The application for the supplement to the work premium may be filed at the same time as you send in your yearly tax return. However, the person who wishes to obtain advanced monthly payments must complete and sign the form available online or at the person’s local employment centre 
.
This form must be returned to the local employment centre. The Ministère de l’Emploi et de la Solidarité sociale will transfer the information on the form to Revenu Québec.
